With ASHP’s PACT (Pharmacy Ambulatory Care Tracker), pharmacists can quickly and easily document and report every patient encounter with detailed intervention data.
PACT is a cloud-based data system designed by pharmacists, for pharmacists. It will help you show the true value of your ambulatory care services by enabling you to swiftly and easily record patient-care in detailed encounter records that integrate seamlessly with the PACT Dashboard.
PACT tracks data logged by clinicians using this mobile app, and will automatically generate essential metrics related to key performance indicators and clinical endpoints.The dashboard has been carefully curated in order to help you demonstrate the value of your ambulatory care pharmacists. Graphical representations of data will give you real time insights to top-level metrics that can be leveraged immediately in reports to your c-suite and other stakeholders.
