If these patients can manage conditions such as diabetes and heart disease better outside of the hospital, costly major medical episodes are less likely to occur. The application could be downloaded an installed on mobile phones, tablets and computers. The Healthcare personnel will register his patients in his accounts. The patients will register their daily parameters in the application and the healthcare personnel will receive alerts about abnormal parameters. The Healthcare personnel can modify their treatment plans, invite them for a consult, send them educational material on their diseases and be reminded when the patients’ control tests are due. The application will also remind patients of their drugs, exercise and diet plans.


Generally, the Medpart will permit the healthcare personnel and the patients to work together for better control and overall success in the management of chronic diseases.
