To get started with the app, you only need your Collective Health login (or your member ID), and the last four digits of your Social Security Number. Start getting the most out of your healthcare!

2 ★ Very buggy. I can't move past fingerprint screen. I've opened tickets for technical issues twice and they are not fixed.
3 ★ I had a survey interview yesterday that was 150.00 that required me to login to the app and because the app was not working properly on my device I couldn't get paid. I reached out to them but never received a reply for that reason I gave them 3 stars
5 ★ Easy access for information, researching providers, filtering search options as well as location options
1 ★ Not able to login after multiple attempts and resetting my password. No error code or information is provided as to why.
