Every time I visit a new doctor, they want me to fill out another long boring medical history form. They all want the same basic information but each form has a different format so you can't just use a copy of the last one you completed.

The worst part is the medications list. You know, writing out all those long nonsensical drug names that are difficult to pronounce and nearly impossible to spell. Of course the doctors needs this information. But honestly, I don't even want to remember those silly names, much less struggle to write them down again.
So, why not have a simple app you can carry around in your cell phone? Instead of copying the same stuff over and over again, enter the information about your medications once to the app and e-mail the list whenever you need. Easier, right?
You bet it is. Worked for me already. Went to a new dentist recently (the old one kept raising his prices). When his receptionist gave me the medical history form, I powered up my phone and sent him my drug list with just a couple of quick clicks, right there in his office. I was prepared. You will be too, after you install this app. Might even come in handy during emergencies.
Welcome to users upgrading from MedList – Free. This Plus version of the app allows you to store med lists for your husband or Mom as well as yourself. You will also find a “Notes” field for each medication giving you a place to store additional information such as the name of the prescribing doctor or the prescription renewal date. And did I mention that this paid version has no ads?

5 ★ I like this app. It shure beats having to write down all those imposible wierd long medication names. it is hard to remember all the names if the medications.

5 ★ I would like to see additional fields for Ordered by/Physician, and Reason For Taking. This would help medical professionals complete their review of my meds....I could just hand them my phone for review.
5 ★ Listmeds makes it so easy to remember all my meds when I am asked by a new doctor or eye doctor for a list I can even email them or print if needed . I even track my vitamins.

5 ★ We are retired and travel away from our home base and physician frequently. We really like this app because it allows us to list each person's d.o.b., physician contact info. and medications individually. In an emergency, we do not have to recall what meds our spouse is taking presently. I would recommend this app to anyone who travels frequently.
5 ★ This app has made my transition from 1 medication to 9 medications overnight a lot more manageable. I simply hand my phone to healthcare person and don't have to try and remember meds and dosages. Great app. Could not be easier to use. Kudos to ListMeds Plus.
1 ★ App does not link to the free one. You can not download what you have already put in to it free. You have to retype everything in to the paid one. Nice app but the free one is better version. But to you do have to put with adds popping up.
4 ★ Hoping the medications input can be recalled when a new device is needed. Loved being able to email my wife the meds list. I'll upgrade to 5☆'s if the inputs can be recovered.
3 ★ I downloaded the free version and it is wonderful. I bought the plus version, but can't seem to upload the free version list into plus version. Any thoughts on how to do this or do I have to retype the whole list? If an "import" is available, then I would rate this ap as a strong FIVE STARS.
2 ★ Update. The application keeps adding partial email address text strings to my name.
5 ★ So very nice to have a current list always handy since my wife's meds keep changing.
