Formstack.com is an easy-to-use, online form builder that allows you to design powerful, secure forms without any programming knowledge. Step 1:
Download the Formstack Mobile Forms app, and create an account on Formstack.com.
Note: Current Formstack customers only need to download the app and log in with their Formstack credentials.
Step 2:
Build your forms on Formstack.com using Formstack's online form builder.
Step 3:
Use the Formstack Mobile Forms app to view your submissions, search for submitted data, and collect new information on your phone or tablet.

1 ★ It's pointless to even try to log in. Crashes all the time. Ughhh!
1 ★ This app is terrible. It crashes more than it works. It used to be stable, but within the last 6 months it hangs when trying to load "my forms," then crashes. Sometimes it hangs while trying to load submissions. Even when it does work it's very slow and frustrating.

5 ★ Stsrted dBusing formstack shortly after launching my business. It has been an indespensible tool all along the way...applications for HR, field management, website forms. Love it. Only criticism is they need other user TNpackages...59 to 249 is a tough leap for most businesses.
3 ★ Started out great, and now the app will not load on my phone. It starts to open and closes suddenly.

2 ★ Mobile app doesn't honor desktop report data. If you delete submissions online they still show as results in the mobile app even after refreshing and clearing cache and data. Meaning if you test your forms and delete the tests, those results will still show in the mobile app.
