ayogoT2D is here to make keeping on top of your diabetes less of a hassle. In your app, you’ll find content related to diabetes management. The personalized content you’ll see will be tailored to you.

More features will be unlocked through regular use of the app— in other words, the more you give the more you’ll get!

Participants will:

1. Receive check-ins and reminders
Sometimes, the app will ask you to review how you’re doing with particular aspects of diabetes management.

2. See educational content
Sometimes card will contain diabetes-related articles, quizzes, videos and stories. Patients that are well-informed tend to have better health outcomes.

3. Connect to services available
There are many government-funded services that are available to you. Taking advantage of them is shown to make a real, tangible difference in people’s health outcomes.

4. Unlock goals & action plans
The goals feature help you create achievable plans that will move you towards achieving your health goals.

5. Earn the diabetes logbook
The logbook helps you keep track of your health data, keep a log of questions that you have for your care team, and see the results of your check-ins and assessments.

ayogoT2D was created by Ayogo Health, in partnership with the Wessex Academic Health Science Network and the Hampshire and Isle of Wight Sustainability and Transformation Plan, and in co-operation with your local GP surgery.

ayogoT2D contains information from multiple trusted sources including: NHS Choices, the Leicester Diabetes Centre and Health Intelligence.
