Our Advanced Forms allow remote data capture on the device using decision based forms (to simplify the process) along with Dropdowns, Signature Capture (some models), and Images to generate the data you need.
App permissions
Actsoft Comet Tracker Demo 1.0.3.4 APK requires following permissions:

Allows using PowerManager WakeLocks to keep processor from sleeping or screen from dimming.

Allows read only access to phone state, including the phone number of the device, current cellular network information, the status of any ongoing calls, and a list of any PhoneAccounts registered on the device.
This constant was deprecated in API level 21. No longer enforced.
