HEALTH - Psoriasis significantly compromised my overall health
PAIN - Psoriasis caused me much pain
ITCH - Psoriasis caused me an unbearable itching
SLEEP - Psoriasis me seriously prevented from sleeping well
SOCIALITY - Psoriasis has seriously affected my social life (going out, going to the gym, pool, restaurant, meet friends, etc.).
WORK AND DAILY ACTIVITIES Psoriasis has caused serious problems in my daily activities (work in the office or at home, study, hobbies, family commitments, etc.).
SERENITY - Psoriasis has seriously affected my serenity
SEXUALITY - Psoriasis made me feel physically unattractive
SHAME - Psoriasis has significantly affected my skin because of its size or because of the sensitive areas affected
SKIN - Psoriasis prevented to show in public and as much as I wanted (in the pool, the gym, the beach, I had to use clothes other than that I wanted to wear)
Instructions for completion:
Answer the questions in the questionnaire in the order in which they are presented.
The time period to which it will refer the user to respond to the ten questions is the last week before the compilation. The answer to each question be indicated by selecting the numerical value on a scale from 0 to 10 where:
0 is equivalent to "" absolutely not "" and 10 equals "" definitely will ""
The points thus identified will be joined by straight lines to form a polygon.
